Vitamin D Levels Associated With Frailty in Women

December 16, 2010 — In older women, both high and low levels of 25-hydroxyvitamin D (25(OH)D) are associated with higher odds of frailty, whereas lower 25(OH)D levels among nonfrail women at baseline were associated with an increased risk for frailty or death after 4.5 years, according to the findings of a prospective study.

Kristine Ensrud, MD, MPH, with the University of Minnesota and the Veterans Administration Medical Center in Minneapolis, and colleagues reported their findings in the December issue of the Journal of Clinical Endocrinology & Metabolism.

Previous studies have reported that frailty predicts the risk for incident disability, falls, fractures, and mortality. However, inconsistent associations have been found between low levels of 25(OH)D and frailty, poor physical activity, increased risk for falls, fractures, and death in older men and women.

In this prospective study, 9704 women 65 years and older were recruited into the Study of Osteoporotic Fractures and were assessed for a period of 12 years for circulating levels of 25(OH)D and frailty status, as defined by 5 criteria: shrinking/sarcopenia (weight loss), weakness, exhaustion, slowness, and low physical activity.

On the basis of the frailty criteria, women were characterized as being robust (none of the criteria), having intermediate frailty (1 - 2 criteria), meeting criteria for frailty (≥ 3 criteria), or having died. Other measurements were also assessed, including vitamin supplementation.

The investigators found that higher odds of frailty were associated with levels of 25(OH)D below 15.5 ng/mL (multivariable odds ratio [OR], 1.47; 95% confidence interval [CI], 1.19 - 1.82), between 15.5 and 19.9 ng/mL (multivariable OR, 1.24; 95% CI, 0.99 - 1.54), or 30 ng/mL or greater (multivariable OR, 1.32; 95% CI, 1.06 - 1.63). As such, unlike previous studies, this association had a nonlinear U-shaped pattern.

Among the 4551 women who were characterized as being robust or having intermediate frailty at baseline, an association was also found between lower levels of 25(OH)D (< 20 ng/mL) and an increased risk for frailty (multivariable OR, 1.21; 95% CI, 0.99 - 1.49) or death (multivariable OR, 1.40; 95% CI, 1.04 - 1.88).

According to the researchers, "inconsistencies between the findings of studies examining the association between 25(OH)D level and frailty status may in part be explained by differences in study populations, sample size, methods to measure 25(OH)D, cut-points used to define 25(OH)D status, definitions of frailty syndrome, or adequacy of adjustment for potential confounders."

However, Dr. Ensrud and colleagues point out that the strengths of their study included "large sample size, prospective analysis, measurement of total 25(OH)D using the highly accurate LC-MS/MS [liquid chromatography tandem mass spectroscopy] method, use of a validated definition of frailty status, and adjustment for multiple potential confounders."

The study was funded by grants from the National Institute on Aging. The study authors have disclosed no relevant financial relationships.

Infeksi saluran pernapasan atas (ISPA) merupakan segala bentuk infeksi yang disebabkan oleh bakteri atau virus. Infeksi saluran napas ini juga bisa terjadi pada bayi baru lahir, terutama bayi yang kekurangan vitamin D.

Gejala yang mungkin timbul akibat ISPA antara lain batuk, pilek, serta demam. Tak jarang juga bayi mengalami muntah dan diare. Semakin cepat ditangani, bayi bisa pulih seperti sedia kala.

Menurut sebuah penelitian, di usia tiga bulan bayi yang kekurangan vitamin D, beresiko dua kali lebih besar menderita ISPA. " Kaitan antara vitamin D dan nafas menciut bisa menjadi gejala penyakit saluran napas, bukan cuma asma tapi juga ISPA," kata Dr.Carlos Camargo, peneliti yang melakukan riset mengenai ISPA pada bayi.

Ia mengatakan meski vitamin D sering dikatikan dengan kesehatan tulang, namun vitamin ini sangat penting untuk meningkatkan sistem kekebalan tubuh.

Dalam penelitian yang dilakukan Camargo diketahui anak yang lahir dari ibu yang cukup vitamin D selama kehamilan lebih jarang mengalami batuk dan nafas berbunyi dibanding bayi dari ibu yang kekurangan vitamin D. Penelitian ini dilakukan terhadap 922 anak yang mengambil bagian dalam New Zealand Asthma and Allergy Cohort Study.

nah... sekarang masalahnya gimana dengan dosisnya? berapa yang dianjurkan? soalnya ini kan vitamin larut lemak yang rada sulit dieliminasi karena cenderung ditimbun di lemak tubuh. jadi ada bahaya akumulasi dan bahkan mungkin toksisitas.

ada dua vitamin D yang dikenal, yaitu ergocalciferol (vit D2) dan cholecalciferol (vit D3). keduanya tidak bioekuivalen dan tidak bisa saling menggantikan. pengujian perbandingan farmakokinetik pemberian pada dosis yang sama menunjukkan dari populasi yang diberikan vit D2 atau vit D3 ditemukan bahwa potensi vit D2 lebih rendah dari vit D3 dan memiliki durasi kerja yang lebih pendek. vit D3 juga lebih direkomendasikan pemakaiannya pada pasien tanpa gangguan ginjal berat, baik sebagai suplemen makanan atau pun pengobatan defisiensi vit D.
